Location, Hours & Contact

We are located at 3100 San Pablo Avenue in Berkeley, between Folger Ave and 67th St., less than a mile from the Ashby Avenue exit of the Eastshore Freeway. If you are using a navigation system like Google maps, Waze, or Apple maps for driving directions, please be sure to include "Berkeley" as the city.

We have free parking (validated, on-site), and easy access by bicycle and public transportation including Ashby BART and AC Transit. 

Building hours are Monday - Friday 8am-5pm. Urgent Care and Imaging Services are also open on weekends and holidays.

Payment Options

Health Insurance
Depending on your plan details, your health insurance plan may not cover part or all of your treatment costs. Please contact your health insurance plan for questions about what’s covered and how much your co-pay amount will be. Please note that we collect Urgent Care co-pay amounts at the time of service.

Self-Pay
If you’re planning to pay for your visit yourself, the office staff will collect the full amount due at the time of your appointment. We offer several convenient ways to pay for your visit, including:
