Home Health Services can accommodate a wide range of care needs. In addition to our skilled nurses, we have Wound Ostomy and Continence Nurses (WOCN) on staff. Additional nurses on staff hold other advanced degrees and specialty certifications in cardiology, oncology and diabetes.
Nutrition consultations with a registered dietitian are available to help your patients with diet and nutrition. Our Medical Social Workers assist your patients and their family with long-term care planning and access to community resources.
Maternal/Newborn/Pediatric Home Care is provided by a special team of registered nurses with experience in the Maternal/Child field including IBC lactation consultants on staff. Our team works with you to give the patients the care, support and information they need to successfully meet the challenges of motherhood with emphasis on education on safety, feeding, growth and development. During the first visit, the nurse conducts a thorough assessment of mother and baby and conducts any blood tests you have ordered. Follow-up visits may be scheduled as needed.
Our highly trained rehabilitation staff of physical, speech and occupational therapists will work with your patients to help them regain and maximize their independence and enjoy a higher quality of life. We advise and assist patients in obtaining all the necessary equipment and supplies for their recovery at home and educate the patient and family throughout the rehabilitative process.
Certified Home Health Aides (CHHAs) perform hands-on patient care to meet your patients needs in accordance with the plan of care under the direction and supervision of one of our licensed professional clinicians. They also assist in maintaining a safe and clean environment. CHHAs are generally scheduled between the hours of 8:00 AM and 4:00 PM. CHHA Duties may include:
