We want to help you to stay healthy after your hospitalization. We know that after your hospital stay, you may not feel 100% and still need extra support and guidance with the instructions you were given by your doctor in the hospital.
Post-Discharge Clinic
The Post-Discharge Clinic is located at the Walnut Creek Outpatient Center. The clinic is for people who have been discharged from the hospital within the past 14 days and have medically complex healthcare issues that need to be addressed right away. Once you are stabilized and recovered, you will then resume your care with your Primary Care doctor and an appointment will be scheduled for you within 2 - 4 weeks.
Our doctors are highly experienced in Internal Medicine or Family Medicine. In addition to working at this clinic, they care for patients at the Walnut Creek Medical Center, Concord Medical Center, and San Ramon Regional Medical Center. The doctors that work in the Post-Discharge Clinic are called Extensivists because they provide an extension of care for patients after they leave the hospital. Extensivists care for patients in multiple locations, such as the hospital, outpatient clinics, Skilled Nursing Facilities, and at home.
How It Works
While you're in the hospital, a nurse will meet you in your hospital room and schedule your appointment within the next 14 days at the Post-Discharge Clinic. The nurse will provide you information about your appointment, directions to the Post-Discharge Clinic, and transportation assistance (i.e. taxi vouchers), if you need it. The Post-Discharge Clinic is a resource to help you optimize your recovery and care after being hospitalized; however it does not change or interfere with your care between you and your Primary Care doctor.
What to Expect at Your Visit
The Post-Discharge Clinic offers you an appointment to see a doctor within 14 days after your hospitalization. The doctor will see you for a one-hour appointment and they will:

Your Primary Care Doctor
The doctors caring for you at the Post-Discharge Clinic are partners with your primary care doctor, which means that your medical care will continue with your own doctor for your next regular appointment and check up. Your Primary Care doctor will also be able to view your medical records from your Post-Discharge Clinic visit.
If you currently do not have a John Muir Health primary care doctor, the hospital nurse will help you select one and schedule an appointment.

Walnut Creek Outpatient Center is located off Highway 680 at Treat Boulevard, between Jones Road and Cherry Lane. Convenient public transportation options include Pleasant Hill BART, which is a short ¼ mile walk, or various County Connection bus lines.
